READ THE INSTRUCTIONS BELOW CAREFULLY

1. Write your name clearly and accurately. The name you enter will be the name used on all Seminary records.

2. Be sure your phone number, postal address, and e-mail address are accurate and complete, as official notices will be sent to you by mail and phone. Also, be sure to fill out all parts of the form.

3. Upload a scanned copy of all certificates [include baptism, marriage register, etc.] and academic credentials which you have. These should all be scanned into one PDF file.

4. Upload one passport photo each of yourself and your spouse (if you are married)

5. For those married but do not have official documentation are required to process them before or by the time of resumption to school.

6. The link here has the recommendation forms you need to print. Write your name and programme clearly at the top of each form before you give them out to those who will recommend you. Give one to each of the following [or the equivalent for a non-Baptist].

[a] Your employer or former school head, which could include Headmaster, Principal, Supervisor, Personal Manager or Head of Department.

[b] Moderator of your Association [apart from your Pastor]. If your Pastor is the Moderator, the Associational Advisor should fill the form.

[c] Your Church Pastor.

[d] You must not sign your own form if you are the Pastor.

[E] Postgraduate students should give the academic reference forms to a lecturer who taught them in their previous program.

NB: Ensure that those recommending you know you personally

These forms should be mailed directly to the school before the application deadline. The Seminary is not responsible for forms not received on time.

Give this to the Church you worship with regularly, to be filled by the Church Secretary and signed and stamped by both the Pastor and Church Secretary after a vote in Church in Conference [date of Church in Conference]. Mail to the Seminary before the application deadline. If you are the Pastor, the Chairman of the Deacons Fellowship could sign.
